Remote or On Site Chart Audits of a maximum of 5% charts for the period or quarter with reviews for completeness of compliance of clinical staff documentation.
a. Physician Documentation Audit
b. Clinical Documentation Compliance Review and Audit
c. HIP AA and CMS Compliance
d. Audit based on the CMS Conditions for Coverage and Various Accreditation
Services documentation guidelines (Example: AAAHC, AAAASF, Joint
Commission, etc.)
e. Reviews based on your specialty. (Example: Pain, Orthopedic, OB-GYN, etc.).
f. Submit a written report for each Audit within 10 business days of the review.
3. Ongoing mentoring in Medical Records including staff education programs as requested.
